You booked the appointment. Ten minutes in, you forgot half of what you meant to say. This worksheet exists so that does not happen again.
What it is
One page. Free, and no email needed. It organizes what you have already noticed about your child, so your doctor gets the full picture in the short time you have together.
How to use it
Fill it in at home, before the appointment, when you are not rushed.
Tick only what you see often, for 6 months or longer, in more than one place, such as home and school.
Bring it to your child's family doctor or nurse practitioner.
That second point matters more than any single behaviour. Every child does some of these sometimes. What counts is whether it has lasted, and whether it shows up in more than one setting. That is the threshold your doctor works from.
What is on the page
The worksheet walks through 5 areas: focus and attention, activity and impulse, mood and behaviour, worry and low mood, and how it is getting in the way at school and at home. There is also space for when you first noticed it, and what you have already tried.
The bottom half is for your physician
Your doctor does not have to hunt for our referral details. The lower section covers how to refer through Ocean eReferral or fax, what is helpful to include such as a recent report card or teacher comments, and confirms that ADHD assessments at Thrive Kids Clinic are covered by OHIP.

What this is not
This worksheet organizes what you have noticed. It is not a diagnostic tool, and it does not replace a clinical assessment. Formal rating scales are completed by parent and teacher, and scored by your physician.
