Yes. A referral is required before we can schedule your baby’s first visit. Can be found here.
You can ask any of the following providers to send us a referral under your baby’s name:
🩺 Your OB/GYN
👶 Your Midwife
🏥 The hospital pediatrician prior to discharge
🧑⚕️ Your family doctor
➡️ Please ask them to fax the referral to: 416-849-2261 (We cannot accept referrals by phone.)
Ask for a referral at the hospital to Thrive Kids Clinic for newborn care - ask for it printed and bring it with you or email it to us at info@thrivekidsclinic.ca
Ask the hospital staff to write “Thrive Kids Clinic” as your baby’s primary care provider on the hospital discharge paperwork.
Give us a call to book the appointment if you are NOT WITH A MIDWIFE
This helps us track your referral and follow up faster!
Great! Midwives provide follow-up care for the first 6 weeks after birth.
Please ask your midwife to:
Fax us your discharge summary after birth
Include a note that you’d like to transfer to Thrive Kids Clinic after 6 weeks
We’ll then call you after the 6-week mark to book your baby’s first visit with us.
You don’t need to call right away — we’ll call you to book your child’s appointment once we receive the referral or midwife discharge paperwork.
However, if you haven’t heard from us within 5 business days, feel free to call us at 416-849-2260 ext 1 (Mon–Fri, Noon–6 PM).
Yes. Your baby must be registered for Ontario Health Insurance (OHIP) before we can book the first visit.
You can apply for newborn OHIP coverage online or at a ServiceOntario location.
