Thrive Kids Clinic is now accepting eReferrals for Newborn and Pediatric Care through Ocean eReferral.

At Thrive Kids Clinic, we work closely with healthcare providers to ensure that every child receives the best possible care. If you are a physician looking to refer a patient to our practice, we would be honored to provide the care they need.

We are dedicated to providing high-quality, comprehensive pediatric care and are committed to working as a team with referring physicians to ensure the best possible outcomes for our patients.

Specialized Referral Form: We are now accepting referrals evaluation consults for medical issues, developmental delay and ADHD evaluations.

No mental health consultations or autism evaluations at this point.

PHYSICIAN REFERRAL FORM

Newborn Referral Form: We are accepting referrals for primary care for newborns under 12 months.

The referral must be under the child's name not the parent. Child must be born.

NEWBORN REFERRAL FORM

Gender Affirming Care: We are accepting patients for Gender Affirming Care.
